He sees the loss of his arm as a form of penance for the anguish and suffering he caused his friends and village. As a result, he turned down Tsunade’s arms made from Hashirama cells.

Will Sasuke Get His Arm Back in Boruto?
In Boruto, Sasuke doesn’t get his arm back. As the Shadow Kage, Sasuke has proven that his missing limb does not prevent him from playing. In the new age, he’s won a lot of bouts without it, yet he still sees his missing arm as a symbol of his wicked past, a recall of why he is on a journey of atonement, and a retribution for the numerous lives he has killed and murdered while being on his road of darkness. There’s no reason to believe he’ll alter his manner shortly, especially when he can already channel his Chidori via his sword.
How Did Naruto Lose His Arm?
After Fourth Shinobi World War, Naruto lost his right arm; Naruto, right-handed, utilized his Rasengan to strike Sasuke’s left-handed Chidori. Because their skill was equivalent, they both lost a component that was eventually supplanted by a fully movable prosthetic arm created by Hashirama Senju’s cells encased in dressings.
Because a prosthetic arm is, per definition, artificial, I’m not clear why it’s prosthetic and built of Hashirama’s cells.
Why Does Sasuke Not Replace His Arm?
Sasuke chose not to have his arm replaced to repent for his transgressions. He sees his missing arm as a relic from his past. Sasuke’s quest for vengeance on and later for his brother Itachi led to the amputation of a limb.
He didn’t think he deserved Tsunade’s gift of a new arm, and as a friend of the Leaf Village (and future Shadow Kage), he needed a continual reminder of how his spiteful instincts could lead him down disastrous roads.
Sasuke has only proved that he can channel his Chidori using his sword; therefore, his left hand isn’t needed.
Does Sasuke Get A Prosthetic Arm?
Sasuke will not be getting a prosthetic arm. He has stayed steadfast in his belief that his missing limb is a remembrance of his past and that he has no plans to obtain a prosthetic arm and is unlikely to do so.
